Undergraduate Petitions & Appeals
A petition is a formal request for an exception to the normal rules and regulations of the Faculty of Music. Requests are considered on an individual basis, and often require supporting documentation.
Undergraduate Music students are strongly recommended to discuss their options with the Registrar or Assistant Registrar prior to submitting a Petition request.
Petitions pertaining to Arts & Science courses should also be submitted to the Faculty of Music Registrar's Office.
Petition Form and Procedure
- You must write a statement, clearly indicating your request, explaining the reasons that support it, and attaching any relevant documentation.
- The Faculty of Music will only accept medical documentation on the Verification of Illness or Injury Form.
- The statement of the reasons should be clear and concise and should be no more than two pages long typed or legibly hand-written. This, along with the supporting documentation, should be submitted to the Registrar’s Office for consideration.
- Note that the petition process is for exceptional circumstances beyond your control and not a means to salvage courses and your academic record, after the fact.
- All students intending to file a petition to the Committee on Academic Standing should consult with the Registrar prior to submitting the Petition Form.

- U of T Verification of Illness or Injury Form
- To be completed only by a dentist, nurse, nurse practitioner, pharmacist at Discovery Pharmacy, physician, psychologist, psychotherapist, or social worker registered and licensed in Ontario or their equivalent in another Canadian province or territory.
- Non-Medical Documentation to Accompany Student Petitions
- (This is not for use by Physicians, Surgeons, Nurse Practitioners, Dentists or Clinical Psychologists. Those practitioners must use the UofT Verification of Student Illness or Injury form.)
